Proof of Identity

If Covered California asks you to confirm your identity, submit a copy of an accepted document. Most people can submit one document, but you have other options if you don’t have a standard ID.

Documents You Can Submit

Submit a copy of one of the following documents:

Government, State or Local ID

  • Driver's license issued by a state or territory
  • Government-issued photo identification card (federal, state or local)

Passport or Foreign Government ID

  • U.S. passport, foreign passport or U.S. passport card. An expired passport may be used.
  • Identification card issued by a foreign embassy or consulate that contains a photograph

School, Military and Voter Records

  • School identification card that includes a photograph
  • Voter registration card
  • U.S. military card, draft record or military dependent's identification card

Citizenship, Immigration or Work Authorization Document

  • Certificate of Naturalization, Form N-550 or N-570
  • Certificate of U.S. Citizenship, Form N-560 or N-561
  • Permanent Resident Card or Alien Registration Receipt Card, Form I-551
  • Employment Authorization Document that contains a photograph, Form I-766

Other Government-Issued Records

  • Native American tribal document with photograph
  • U.S. Coast Guard Merchant Mariner card
  • A verified identity record from a federal or state government agency — including public assistance, law enforcement, tax, or corrections agencies. The record must confirm the agency has verified and certified your identity.

Documents for Children Under 16

For a child under age 16, submit one of the following:

  • A clinic, doctor, hospital or school record
  • If none of those records is available: a written statement signed under penalty of perjury by the parent or guardian stating the child's date and place of birth.
    • This statement cannot be used together with a citizenship affidavit because a citizenship affidavit and a birth statement serve the same purpose and cannot substitute for each other.

If You Don’t Have One of These Documents

If you cannot provide any of the identity documents listed above, you must provide copies of two of the following documents:

  • Birth certificate
  • Social Security card
  • Marriage certificate
  • Divorce decree
  • Employer identification card
  • High school or college diploma (including high school equivalency diplomas)
  • Property deed or title document
  • Adoption decree
  • Foreign school record that contains a photograph
  • Notice from a public benefits agency
  • Union or worker's center identification card
