You can submit documents online through your account, by fax, by mail or in person with free help from a certified enroller or insurance agent.

Before You Submit

A few things to keep in mind before you send your proof:

  • Submit only the document type your notice or account message asked for — not all documents are accepted for every request.
  • Send copies only. Do not send original documents by fax or mail.
  • Make sure your document is clear and includes the information requested.
  • Keep your notice and case number available in case you need to follow up with Covered California.

Why Documents Are Requested

When you apply, Covered California checks the information you provide against government data sources, such as the IRS and Social Security Administration. If something doesn't match, we may ask you to send proof to confirm the new information. This doesn't necessarily mean you made an error — it's a routine part of the eligibility process.
